How long does withdrawal from substances last?

Withdrawal length varies by substance and by person. Acute alcohol and benzodiazepine withdrawal typically begins within hours of the last dose and, at its most severe, peaks in the first forty-eight to ninety-six hours. Opioid withdrawal usually begins eight to twenty-four hours after the last dose, peaks in thirty-six to seventy-two hours, and largely resolves within five to ten days. Stimulant withdrawal is primarily psychological and can last one to two weeks acutely. Post-acute withdrawal syndrome, with lingering mood, sleep, and cognitive symptoms, can extend for weeks to months in any of these categories. Is withdrawal dangerous?

Some withdrawals are medically dangerous. Alcohol and benzodiazepine withdrawal both carry real risks of life-threatening seizures and, in severe cases, delirium, and should never be attempted without medical supervision. Opioid withdrawal is rarely fatal on its own but is intensely uncomfortable and reliably drives relapse without clinical support. Fentanyl withdrawal is similar but more complex and less predictable due to tissue accumulation. Stimulant withdrawal is primarily psychological but can include severe depression and suicidal ideation. Never stop a substance you are physically dependent on without medical guidance first.

Can you manage withdrawal at an outpatient center?

Medical detox happens at an accredited inpatient or residential facility, not at our outpatient centers. What helps with withdrawal symptoms?

Medically supervised detox is the safest approach to managing withdrawal from alcohol, benzodiazepines, or opioids. In a supervised setting, approved medications can ease specific symptoms: benzodiazepines for alcohol withdrawal seizure prevention, buprenorphine or methadone for opioid withdrawal, clonidine for cardiovascular opioid symptoms, and supportive care throughout. After stabilization, medication-assisted treatment and therapy address the cravings and underlying causes that follow.
