The Continuum of Care

The continuum of care is the sequence of treatment levels a person moves through during recovery, each one less intensive than the last. A typical path runs from medical detox to a Partial Hospitalization Program (PHP), then to an Intensive Outpatient Program (IOP), then to standard outpatient care, and finally to aftercare and alumni support. The idea, formalized in the levels-of-care framework from the American Society of Addiction Medicine (ASAM), is to match the intensity of treatment to where a person actually is, then reduce that intensity gradually so recovery skills have time to take hold. The Archangel Centers provides the outpatient levels of this continuum (PHP, IOP, and OP) and coordinates detox and inpatient placement with licensed partners when a higher level is needed.

Why the continuum exists

Recovery is rarely a single event. Stopping substance use, stabilizing the body and mind, building coping skills, and returning to daily life each demand a different level of support. A continuum of care lets treatment intensity rise and fall with real clinical need instead of forcing everyone through the same fixed program.

Research on addiction treatment consistently points to the same lesson: people who step down gradually through connected levels of care tend to maintain recovery more reliably than those who leave abruptly after a single phase. The Substance Abuse and Mental Health Services Administration (SAMHSA) similarly emphasizes a continuum of care and continuing support as central to lasting recovery. The continuum is built to prevent that abrupt drop-off.

Detox: the coordinated first step

When stopping a substance carries medical risk, treatment usually begins with detox, a short, medically supervised period of withdrawal. The Archangel Centers does not provide detox or inpatient care on site. Instead, our team coordinates placement with licensed detox partners, manages the clinical handoff, and holds a place in our outpatient programming so there is no dangerous gap between detox and ongoing treatment.

Timing matters here. The days right after detox carry the highest relapse risk, so moving directly into structured outpatient care is a core part of how the continuum protects people.

PHP to IOP to outpatient: stepping down

Once a person is medically stable, the outpatient continuum carries them forward in stages.

  • PHP (ASAM Level 2.5): about 25 to 30 hours a week of structured day treatment
  • IOP (ASAM Level 2.1): about nine or more hours a week, flexible around work and family
  • Standard outpatient: one to a few hours a week, focused on continuity and accountability
  • Each step keeps the same treatment team where possible, so care stays continuous

Aftercare and long-term support

The continuum does not end when structured treatment does. Aftercare connects people to continuing therapy, alumni networks, and community recovery resources so support remains available after formal programming winds down. A strong aftercare plan is one of the most reliable predictors of lasting recovery.

No. The Archangel Centers provides outpatient levels of care: PHP, IOP, and standard outpatient treatment. When detox or inpatient care is medically necessary, our team coordinates placement with licensed partner facilities and manages the transition into our outpatient programs once a person is stable.

Do I have to start at the top of the continuum?

Not necessarily. The right entry point depends on a clinical assessment of medical stability, the severity of the condition, home environment, and history. Some people start with detox, others begin directly in PHP or IOP, and some enter at standard outpatient care. The continuum is a map, not a fixed sequence everyone must follow.

Can I move back up a level if I need to?

Yes. The continuum works in both directions. If someone struggles or relapses at a lower level, the clinical team can step the intensity back up, for example from outpatient to IOP, to provide more support before easing off again.

What happens to my care when I move between levels of the continuum?

At The Archangel Centers, stepping between PHP, IOP, and standard outpatient care keeps the same treatment team wherever possible. Your therapist, relapse-prevention plan, and progress notes carry forward, so each transition builds on what came before rather than starting over. This continuity is one of the main reasons connected step-downs support recovery better than abrupt exits.

Does the continuum of care work for co-occurring mental health conditions?

Yes. The Archangel Centers treats substance use and co-occurring conditions such as anxiety, depression, or trauma together throughout the continuum. Psychiatric services and medication management are woven into each outpatient level, so mental health support does not stop when treatment intensity steps down. Addressing both at once is central to durable recovery in New Jersey and North Carolina.
