How we use and disclose your health information

We use and disclose your PHI for the purposes permitted by HIPAA, primarily treatment, payment, and health care operations.

• Treatment: to provide, coordinate, and manage your care, including coordinating placement with other providers when a higher level of care is needed.
• Payment: to bill and obtain payment for the services you receive, including verifying insurance benefits.
• Health care operations: for quality improvement, training, and the administrative activities required to run our programs.
• As required by law: when disclosure is mandated by federal or state law, or to prevent a serious threat to health or safety.

Your rights

HIPAA gives you specific rights over your protected health information. You may exercise these rights by contacting us in writing.

• The right to access and obtain a copy of your health records.
• The right to request a correction to information you believe is inaccurate or incomplete.
• The right to request restrictions on certain uses and disclosures.
• The right to request confidential communications through a specific channel or location.
• The right to an accounting of certain disclosures of your information.
• The right to receive a paper copy of the official Notice of Privacy Practices.

Special protections for substance use records

Because The Archangel Centers provides substance use and mental health care, certain records may receive heightened confidentiality protections under federal regulations in addition to HIPAA. In general, these records are not disclosed without your written consent except in limited circumstances permitted by law.

How to file a complaint

If you believe your privacy rights have been violated, you may file a complaint with The Archangel Centers and with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
