How we use and disclose your health information
We use and disclose your PHI for the purposes permitted by HIPAA, primarily treatment, payment, and health care operations.

- The right to access and obtain a copy of your health records.
- The right to request a correction to information you believe is inaccurate or incomplete.
- The right to request restrictions on certain uses and disclosures.
- The right to request confidential communications through a specific channel or location.
- The right to an accounting of certain disclosures of your information.

Special protections for substance use records
Because The Archangel Centers provides substance use and mental health care, certain records may receive heightened confidentiality protections under federal regulations in addition to HIPAA. In general, these records are not disclosed without your written consent except in limited circumstances permitted by law.
How to file a complaint
If you believe your privacy rights have been violated, you may file a complaint with The Archangel Centers and with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
