PHP vs IOP

PHP and IOP are the two most intensive outpatient levels of addiction and mental health treatment, and people often weigh one against the other. A Partial Hospitalization Program (PHP) delivers roughly 25 to 30 hours of structured care each week and sits at ASAM Level 2.5. An Intensive Outpatient Program (IOP) delivers about nine or more hours each week at ASAM Level 2.1. These levels come from the American Society of Addiction Medicine (ASAM), which publishes the standard framework for matching treatment intensity to clinical need. Both let a person live at home rather than at a facility. The main difference is intensity: PHP is closer to full-day treatment, while IOP is built to fit around work, school, and family. The clearest way to tell PHP and IOP apart is the weekly time commitment. PHP runs five to six days a week, several hours a day, totaling about 25 to 30 hours. IOP typically runs a few sessions a week, totaling around nine or more hours. That difference in hours is what makes PHP suited to early, higher-need recovery and IOP suited to people who are more stable or transitioning back to daily life.

The decision is clinical, not personal preference. A licensed assessment weighs medical stability, the severity of the substance use or mental health condition, the safety of the home environment, co-occurring conditions, and recent treatment history.

Is PHP better than IOP?

Do most people go from PHP to IOP?

Many do. Stepping down from PHP to IOP is a common path because it keeps care continuous while gradually reducing intensity. Stepping down through connected levels tends to support recovery more reliably than leaving treatment abruptly after a single phase.

Can I start with IOP instead of PHP?

Yes, if a clinical assessment finds it appropriate. IOP is a valid entry point for people who need more than weekly therapy but do not require full-day programming. It is not required that someone complete PHP first.

Does insurance treat PHP and IOP differently?

Insurers authorize PHP and IOP separately because they are distinct levels of care under the ASAM Criteria, and PHP carries more clinical hours. Most plans, including the Blue Cross Blue Shield plans common in New Jersey and North Carolina, cover both when medically necessary.
